4Korners, in collaboration with la FADOQ – Région des Laurentides, is currently conducting a needs assessment to better understand the needs of people who consider themselves to be seniors.
We would like to understand all needs you may have, specifically in relation to health, social services and other related services offered in your area. The purpose of this survey is to determine whether the existing services offered in the Laurentians (from all over the Laurentians: St-Eustache to Mont-Laurier and Rosemère to Oka) meet the needs of the English-speaking community.
Any information we gather during the survey will be kept strictly confidential and only authorized personnel at 4Korners will have access to it.
We will begin by learning about you, then see if you know about programs offered in English, in your area. We will finish by understanding what your needs are now and what you anticipate them to be in the future.
